Abstract
The transition from the hospital to the home environment is very stressful for the child and his parents, especially if the child has a chronic disease and an inserted tunnelled central venous catheter, on which his life depends, because it basically sustains life. The process of discharge of the child from hospital is very complex. The process involves various professionals who also work with the child and his family after discharge from hospital. Therefore, it is necessary to prepare a specific plan for the care of the child with a tunnelled central venous catheter in the home environment. The paper presents the phases that are necessary for discharge home of the child with the short bowel syndrome. The nurse who cares for the child plays an important role in confronting parents with the child‘s chronic disease. The nurse leads them with her professional attitude and expertise to the final goal. The educational process for safe and quality care of children with chronic illness at home is adjusted to each family.
